About The Role

Directorate: Strategic Commissioning and Investment, Adult Social Care

Job Title: Community Equipment Officer

Grade:  PO2

Are you looking for a new and exciting opportunity to work in Adult Social care commissioning? Are you passionate about ensuring that people receive the right support to live as independently as possible?

Context

We have a shared strategic ambition to support people in Islington to Start, Live and Age Well. We take a positive approach to fostering wellbeing through early intervention and prevention, with consideration for the wider determinants of health, strengths-based approaches and integration across health and care. We are committed to working with residents and partners to ensure residents are able to live the lives they want, with as much independence as possible.

An opportunity has arisen for an individual to join the Islington Adult Social Care (ASC) Strategic Commissioning team as a Community Equipment Officer.

This is a new and unique role that supports a system wide approach across Health and Social Care to ensure a good quality and cost-effective community equipment service.

This role involves being responsible for monitoring the performance of our community equipment contract and identifying and taking action to address trends in service performance, including improvements in quality and areas of concern.

This is an exciting role that will involve working with acute hospital colleagues, as well as a range of community health and social care professionals who prescribe community equipment. You will also work closely with the community equipment service provider and the London Consortium lead borough to drive continual service improvements.

This role is advertised as a 12-month fixed-term contract.

Candidates for the role should have relevant contract management experience within a social care or other relevant environment and possess a strong understanding of social care systems.
