Transition Social Worker

A new and exciting opportunity has arisen for an experienced social worker to work in partnership with service users with global learning disabilities, their families and carers and local service provider agencies whilst going through the transition to Adulthood.

The successful applicant will be working with service users with profound and complex physical and mental health needs and will be based within the multidisciplinary health and social care Transition Team at ILDP.

You will be responsible for undertaking the Adults assessment, accessible support planning and review of service user’s needs to enable them to achieve their potential.

We aim to support people with learning disabilities towards achieving greater autonomy and independence within the community. Experience of personalisation is therefore essential.
