Senior Practitioner - Occupational Therapist

We are looking for an experienced and enthusiastic senior practitioner occupational therapist to lead Islington’s Discharge to Assess Single Point of Access Team. Over the last 12 months, Islington Adult Social Care department has been developing a ‘discharge to assess’ (D2A) offer to help people who are ready to be discharged from hospital, but who still need some support, to get home more quickly by assessing their support needs in the community.

The Single Point of Access (SPOA) are a multi-disciplinary team providing the interface between our hospital partners and adult social care in the D2A project. Demonstrating excellent management skills, the senior OT will proactively lead the SPOA team and play a critical role in the operational delivery of D2A, providing support and supervision and a point of escalation for more complex case discussions to other professionals in the team.

In addition to this team management and clinical oversight, the post holder will be a central point of contact for senior colleagues working in local acute hospitals and as such have a high profile with key stakeholders. This will require an ability to demonstrate excellent management skills in proactively leading the team.

The post will be integral to the ongoing strategic development and scaling up of the project working with partners across the North Central London STP footprint. The post holder will play an active role in Islington’s approach to the national transformation agenda underway to improve flow in hospitals and deliver better outcomes for patients transferring to social care. There will be significant opportunities for joint working with professionals from other services both within and beyond the borough.

There are excellent career development opportunities available in this role and as D2A enters its first full year in operation, there is a real opportunity for the post holder to shape, develop and improve the service design.
