We are looking for an experienced administrator to provide high level business and project support to Islington’s Discharge to Assess Single Point of Access Team. Over the last 12 months, Islington Adult Social Care department has been developing a ‘discharge to assess’ (D2A) offer to help people who are ready to be discharged from hospital, but who still need some support, to get home more quickly by assessing their support needs in the community.

The Single Point of Access (SPOA) are a multi-disciplinary team providing the interface between our hospital partners and adult social care in the D2A project. Delivering high quality administrative support, the post holder will play a key role in supporting professionals in the SPOA team to manage work flow and ensure business processes are carried out efficiently to support the smooth transition of patients from hospital to social care services.

In addition to office support, the post holder will also take a lead in a number of aspects of the ongoing project work underway to further scale up and improve D2A in the borough. This will include supporting the roll out of our service user engagement plan, maintaining the evaluation database for the project and attending/coordinating project meetings where relevant.
