North Central London Transforming Care Partnership is excited to be setting up a new pilot service to support families who are part of the Transforming Care cohort. The Transforming Care Prevention and Support Service will operate across two patches; Islington, Haringey and Camden, and Barnet and Enfield.

This new service aims to support families before they reach crisis point, in a personalised way to help them develop resilience and better cope with the complexities of looking after a disabled child with behavioural challenges and, frequently, a range of other difficulties. The aim is keep children and young people in the community, and avoid admission to Tier 4 hospital placements and 52-week residential placements.

This advert is for the service which will operate across Islington, Haringey and Camden, and will be based within the Disabled Children’s Family Support Team in Islington.

You will be providing direct support to children with complex disabilities and their families in a range of settings: in their homes, in the community, and in liaison with their short breaks setting, their school and mental health services. You will also proactively contribute toward the team around the child, including safeguarding children’s welfare and increasing their quality of life.

Having identified with the family the most important areas where they would benefit from additional support, you will work with them intensively to enable positive change and achieve the agreed target outcomes.

You will provide focused, high quality support to the children and their families. The target outcomes you agree will be informed by and contribute to; education, health and care plans, child in need plans, mental health care plans and positive behaviour support plans.

The range of areas in which you will support children, young people and their families will include: maintaining physical health, meeting emotional needs, keeping children safe, connecting with social support networks, accessing education and learning, maintaining boundaries and reducing challenging behaviour, managing family routines, maintaining home and parental resilience.

The ideal candidate will have a background in health, education and/or social care, working closely with families with complex needs. Key strengths of the Key Workers will be flexibility, a focus on outcomes, a willingness to try new approaches and an inclusive approach. This service puts the family at the heart of the service and will work alongside other teams and professionals to ensure a joined-up approach.

There are 3 Key Worker working across Haringey, Islington and Camden. The roles are all full-time roles for 12 months, and there is an expectation that the working hours will be flexible, including evenings and weekends, as required by families.
