Admission Avoidance Case Manager

Islington’s Single Point of Access (SPOA) Team are recruiting a case manager to play a key role in an innovative Admissions Avoidance service pilot for vulnerable Islington residents. The service offers support for patients who have received effective clinical support in the community to stop them being admitted to hospital but who require some social care support to regain their independence at home.

As an admissions avoidance case manager, you will work with service users at home to identify their strengths and goals to inform an assessment of their care and support needs. With responsibility for a caseload, you will support service users to progress towards their identified goals, working closely with care providers and the wider multi-disciplinary team to adjust and step down support as required. In addition, you will also provide support where required to the wider SPOA and reablement team.

With knowledge and experience in carrying out Care Act assessments, setting reablement goals, assessing service user and environmental risks and completing documentation to a high standard, you will also play in important role in liaising with health partners including GPs and hospitals, referring to therapists, and signposting to other services such as telecare and partners in the voluntary and community sector.

As this is a pilot project, there will be a number of opportunities to contribute to project development and evaluation processes as the service develops. This is a really exciting time to join the SPOA and play a key role in extending and improving the community support available to Islington residents.
